About the Role 

We are hiring a Physical Therapist to join our Ortho-Based Home Care Transition Program in partnership with Luminis Health. This innovative care model focuses on short-term in-home rehab—typically 2–3 visits—prior to transitioning patients to outpatient care. This role offers a unique alternative to traditional home health: 

  • No SOC (Start of Care) visits 
  • No case conferences or OASIS documentation 
  • Orthopedic caseload only 
  • Focus strictly on evaluation and follow-up visits 

You’ll see approximately 6–7 patients per day with a goal of 30 visits per week.

License & Experience

 

  

Requirements for this position include the following:  

  • Graduate (current or pending) from an accredited physical therapy program  
  • Active or pending state physical therapist license 
  • Comfort working independently in the home setting
