You’ll be part of the therapy specific team supporting Home and Heart Home Health Care Agency, working within a collaborative, multidisciplinary model. Care is coordinated, patient‑centered, and team‑driven, with shared accountability and open communication.

Job Overview & Work Site

 

Your Patients 
You’ll work with a diverse caseload, including a higher‑than‑average post‑operative orthopedic population.

You’re set up to do your best work through: 
 Thoughtful scheduling and defined territories to support appropriate caseloads 
• Limited drive time between visits whenever possible 
• Clinical leadership from therapists who understand day‑to‑day practice demands 
• Individualized support aligned with your clinical interests and goals 
• Collaboration with Home and Heart healthcare colleagues for a team‑based approach to care

License & Experience

 

Position Requirements 
• Graduate (current or pending) of an accredited occupational therapy program 
• Active or pending state occupational therapist license 
• New graduates encouraged to apply
